Do you accept insurance?
At this time, Pure Health Primary Care Clinic is a self-pay clinic and does not accept insurance. Payment is due at the time of service.
Do you accept HSA or FSA cards?
Yes, HSA and FSA cards are accepted for eligible services.
Do you treat children?
Yes, we see patients from ages 12 months and up.
Will I receive a receipt or superbill for insurance reimbursement?
Yes, a receipt can be provided. If needed, we may also be able to provide documentation you can submit to your insurance for possible out-of-network reimbursement. Reimbursement is not guaranteed and depends on your individual insurance plan.
